    A. Electronic Passport (E-Passport).
                   (Minimum of 7 to 10 working days of processing)                               Expedited P 2,500.00
                              Personal appearance is required regardless of what age.

 

 

 

LOST PASSPORT ASSISTANCE

 

 

REQUIREMENTS:

 

     1.   Affidavit of Lost (Notarized)

     2.   Birth Certificate (NSO)

     3.   Police Report

     4.   Valid ID’s:  

                a.  SSS ID

                b.  Driver’s License

                c.  Voter’s ID

                d.  PRC ID

                e.  Marriage Certificate

 

  NOTE:         Lost Passport considered as First Timer Applicant

 

  Processing:          1 Month ( If passport is still Valid)

                                15 days (If passport is already expired)

 

                                    Service Fees:      Php. 2500 per person
