Initial Requirements:

• Copy of first page of the valid passport at least 6 months prior to departure.

• Copy of previous US Visa (if any)

• Draft of DS-160 Application (to be requested)

• 1PC, 2x2 Picture with white background.

• Copy of receipt of US Visa fee from the Bank (validation date and numbers must be visible).

US visa fee must be paid at any branch of Bank of the Philippine Island (BPI)

 

 

GENERAL VISA REQUIREMENTS   (FOR USA & CANADA )
 
 ___ Passport valid for at least 6 months from date of    Travel.  Include old passports if any. 
___ 2 pcs. Passport size picture (White Background)         (2x2 cut for US & JAPAN)
 
___ Evidence of Financial Capability   
·         Original Bank Certificate of Savings / Time / Dollar deposits
·         Photocopy of Bank Book / Time deposit certificate
·         Bank Statements for the last 3 months for the current accounts
·         Photocopy of Credit card statements for the last 3 months if any
·         Photocopy of Land Title / Car Registration if any
 
___ Business Registration from SEC / DTI

        Or Mayors Permit (if in business)   
___ Certificate of Employment and Leave
        Of Absence (if employed). 
___ Latest Income Tax Returns 
___ Authenticated Marriage Contract (if married) 
___ Authenticated Birth Certificate (if single) 
___ Photocopy of Professional ID (PRC,IBP etc.)
 
 ADDITIONAL REQUIREMENTS:
FOR MINOR /STUDENT APPLICANTS
___ Authenticated Birth Certificate from NSO 
___ Authenticated Marriage Contract of parents
       From NSO 
___ Affidavit of Support and Consent if traveling
        With only one or neither of the parents 
___DSWD Clearance if traveling with only or
       Neither of the parents 
___PROOF OF SCHOOLING (if studying)
       Original School Certificate / Photocopy of I.D.

 FOR CONVENTION TRIPS
___ Certificate of membership from the organization 
___ Confirmation of convention registration if any 

FOR BUSINESS TRIPS
___ Letter of Invitation from the company abroad 

FOR SPONSORSHIP TRIPS
___ Evidence of funds from the inviting party
        (Eg. business registration, bank certificate of
        Deposits, ITR etc.) 
___ Affidavit of Support from sponsoring party 
___ Letter of Invitation 

FOR GOVERNMENT EMPLOYEE
___ Travel Authority
