VISA REQUIREMENTS (FOR CANADA)

1.  Duly Accomplished Application Form

2.  Photocopy of any Valid ID

3.  2 pieces Passport Size Photo (4.5 x 3.5 cm) in White Background.

4.  Valid Passport (must be valid within 6 months from the date of departure) include old passport if any.

5.  Original Bank Certificate with photocopy of Bank Statement, Passbook or Time Deposit Certificate

6.  Original Employment Certificate indicating salary, position and Leave of Absence (IF EMPLOYED

7.  Photocopy of SEC or DTI and Mayors Permit (IF IN BUSINESS)

8.  Photocopy of Income Tax Return

9.  Photocopy of Credit Card Statement (if any)

10. Photocopy of OR/CR and Land Title (if any)

11. Photocopy of ACR or ICR (FOR FOREIGN NATIONALS)

12.  Photocopy of IBP or PRC (FOR PROFESSIONALS)

13.  Photocopy of Birth Certificate (IF SINGLE/MINOR)

14. Photocopy of Marriage Certificate (IF MARRIED)

15.  Original School Certificate and Photocopy of School ID (IF SCHOOLING)

16. Notarized Affidavit of Support from Parents/Host (IF MINOR/DEPENDENT)

17. (IF INVITED BY RELATIVE or FRIEND)

             *   Photocopy of the sponsors Passport

             *   Photocopy of Residence Card

             *   Photocopy of Employment Certificate
